ABSTRACT

Objective @#To examine the diagnostic and prognostic value of long non-coding RNA (lncRNA) JPX in mesothelioma, so as to provide insights into diagnosis and prognosis of mesothelioma. @* Methods@# Patients with clinically definitive diagnosis of mesothelioma from 2015 to 2019 that were sampled from asbestos processing plants in Zhejiang Province from 2015 to 2019 were recruited in the mesothelioma group, while healthy residents without asbestos exposure or asbestos-related diseases in the same area served as controls. Participants' demographics, pathologic diagnosis and imaging features were collected, and the expression of blood lncRNA JPX was detected using lncRNA microarrays. The diagnostic value of lncRNA JPX for mesothelioma was evaluated using the receiver operating characteristic (ROC) curve, and the correlation between lncRNA JPX expression and prognosis was examined among mesothelioma patients using survival analysis. @* Results@# There were 17 subjects in the mesothelioma group, with a mean age of (65.71±8.36) years, and 34 subjects in the controls, with a mean age of (64.24±8.70) years. LncRNA microarray detected significantly high lncRNA JPX expression in mesothelioma patients, and higher blood lncRNA JPX expression was detected in the mesothelioma group than in the control group [median (interquartile range), 1.10 (1.31) vs. 0.89 (0.54); t'=-2.300, P=0.034]. The area under the ROC curve was 0.673 (95%CI: 0.507-0.839, P=0.046), and if the cutoff was 1.759, the sensitivity and specificity were 35.3% and 100.0%, respectively. Survival analysis showed no significant difference in the survival rate of mesothelioma patients between the high lncRNA JPX expression group and the low expression group (χ2=0.212, P=0.645). @*Conclusions@# LncRNA JPX overexpression is detected in the blood of patients with mesothelioma, and lncRNA JPX expression presents a diagnostic value for mesothelioma; however, it shows little prognostic value for mesothelioma.

RESUMEN Objetivo. Establecer la carga de enfermedad por mesotelioma maligno (MM) en Colombia entre 2015 y 2020 y su asociación con el índice sociodemográfico (SDI) subnacional y las localizaciones de asbesto. Métodos. Estudio ecológico mixto en la población colombiana con diagnóstico de MM según la CIE-10 durante 2015 a 2020. La carga global de enfermedad (GBD, por su sigla en inglés) se estimó por medio de la metodología propuesta de Murray y López a partir de la prevalencia y mortalidad obtenida de fuentes oficiales. Se estimó el SDI (por su sigla en inglés) subnacional (nivel departamental) como medida de desarrollo socioeconómico y se establecieron regresiones lineales con la GBD, el SDI y las localizaciones documentadas de asbesto. Resultados. La GBD estimada por MM en Colombia durante 2015-2020 fue de 51,71 años de vida ajustados por discapacidad (AVAD) por cada 1 000 000 de habitantes (15 375,79 AVAD totales), con predominio en personas mayores de 50 años (91,1%) y de sexo masculino (66,4%). A nivel departamental, Bogotá y Valle del Cauca presentaron la mayor cantidad de AVAD ajustados; mientras que Bogotá tuvo el SDI más alto, y Guainía y Cesar el más bajo. Se evidenció una asociación entre los AVAD y el SDI, donde este último explicó 22,8% de los casos de AVAD. Conclusión. El MM es causa de una gran cantidad de AVAD, con predominio en los departamentos con mayor desarrollo socioeconómico, y con presencia de empresas que solían utilizar asbesto; no obstante, el posible subdiagnóstico de MM limita el análisis de la información.


ABSTRACT Objective. Establish the disease burden of malignant mesothelioma (MM) in Colombia between 2015 and 2020, and its association with the subnational sociodemographic development index (SDI) and with asbestos sites. Methods. Mixed ecological study of the Colombian population diagnosed with MM (according to ICD-10) from 2015 to 2020. The global burden of disease (GBD) was estimated using the methodology proposed by Murray and Lopez, based on prevalence and mortality data obtained from official sources. The subnational (departmental level) SDI was estimated as a measure of socioeconomic development. Linear regressions were established with the GBD, SDI, and documented asbestos sites. Results. The estimated GBD of MM in Colombia during 2015-2020 was 51.71 disability-adjusted life years (DALYs) per 1 000 000 inhabitants (15 375.79 total DALYs), with predominance in people over 50 years of age (91.1%) and males (66.4%). Bogotá and Valle del Cauca were the departments with the highest number of adjusted DALYs. Bogotá had the highest SDI and Guainía and Cesar had the lowest. There was evidence of an association between DALYs and SDI, explaining 22.8% of DALYs. Conclusion. Malignant mesothelioma is the cause of a large number of DALYs, predominantly in the departments with greater socioeconomic development and with companies that used to use asbestos. However, possible underdiagnosis of MM limits analysis of the information.


RESUMO Objetivo. Estabelecer o ônus da doença por mesotelioma maligno (MM) na Colômbia entre 2015 e 2020 e sua associação ao índice sociodemográfico subnacional (ISS) e locais de amianto. Métodos. Estudo ecológico misto na população colombiana diagnosticada com MM, de acordo com a CID-10 durante 2015 a 2020. A carga global da doença (CGD) foi estimada usando a metodologia proposta por Murray e López com base na prevalência e na mortalidade obtidas de fontes oficiais. O SDI subnacional (nível departamental) foi estimado como uma medida de desenvolvimento socioeconômico e foram estabelecidas regressões lineares com CGD, SDI e localizações documentadas de amianto. Resultados. A estimativa de CGD por MM na Colômbia entre 2015-2020 foi de 51,71 anos de vida ajustados por incapacidade (AVAI) por 1 000 000 de habitantes (15 375,79 AVAI totais), com predominância em pessoas com mais de 50 anos (91,1%) e do sexo masculino (66,4%). Com relação aos departamentos, Bogotá e Valle del Cauca tiveram o maior número de AVAI ajustados, enquanto Bogotá teve o maior SDI, e Guainía e Cesar, o menor. Houve uma associação entre os AVAI e o SDI, sendo que o SDI foi responsável por 22,8% dos AVAI. Conclusões. O MM é a causa de um grande número de AVAI, predominantemente em departamentos com maior desenvolvimento socioeconômico e com a presença de empresas que usavam amianto; no entanto, o possível subdiagnóstico do MM limita a análise das informações.

ABSTRACT

Malignant mesothelioma of the tunica vaginalis testis (MMTVT) is a rare tumor. At present, there are still many disputes in its epidemiology, pathogenesis, selection of diagnostic methods, treatment and prognosis. Asbestos exposure, ionizing radiation and chromosome abnormalities are the risk factors of MMTVT. Immunohistochemistry, ultrasonography and electron microscope can be used for the diagnosis and aggressive surgery is the main treatment method. The development of endoscopic surgery, multi-disciplinary treatment (MDT), tumor targeted therapy and immunotherapy will bring more benefits to MMTVT patients.

ABSTRACT

Asbestos, one among the inosilicate group of minerals is listed under the Sikatha varga (Silicate compounds) in the Rasasastra treatises, in the name, Kousheyashma. Though the asbestos is vastly used for commercial purposes, it is not found to be used in clinical practices owing to its toxicity. But in Ayurveda, Acharyas have opined that by the judicious usage, even the poison can be bestowed medicinal value. And hence, after undergoing appropriate pharmaceutical processes as mentioned in the treatises, Kousheyashma can be effectively administered in Bhasma (after incineration) form in the treatment of various ailments like leucorrhoea, painful micturition, inflammation of urinary tract, menorrhagia. In this work, an attempt is made to expose the hidden medicinal values of asbestos by converting it into Bhasma form. Kousheyashma is a very cost effective and potent medicine which could be effectively used by Ayurvedic practitioners. In the present work, conversion of raw Kousheyashma into medicinal Bhasma form is discussed elaborately. The processes involved includes Sodhana and Marana of Kousheyashma. The liquid media used for the purpose of Sodhana was Gomutra and for Marana purpose, Harithamanjari swarasa was used. Three Putas were required to obtain Kousheyashma bhasma which satisfied the Bhasma pareekshas.

Objective @#To analyze the survival of patients with malignant mesothelioma, so as to provide insights into the management of malignant mesothelioma.@*Methods @#Totally 36 patients with malignant mesothelioma admitted to Cixi Third People’s Hospital from October 2012 to January 2021 were enrolled, and the demographic features, exposure to asbestos, and diagnosis and treatment were retrospectively reviewed. The survival rate and median survival time were calculated with the life-table method, and the factors affecting the survival rate of malignant mesothelioma were identified using the Kaplan-Meier estimate and log-rank test.@*Results @#The 36 patients with malignant mesothelioma included 6 men ( 16.67% ) and 30 women ( 83.33% ), and had a median age of 61 ( interquartile range, 14 ) years. There were 30 cases with pleural malignant mesothelioma ( 83.33% ) and 6 cases with peritoneal malignant mesothelioma ( 16.67% ), 32 cases ( 88.89% ) with a history of occupational exposure to asbestos, and 26 cases ( 72.22% ) receiving palliative treatment. The 1-, 2- and 3-year cumulative survival rates were 30%, 15% and 3%, respectively, and the median survival time was 0.71 years. In addition, there were no significant differences in the survival period among patients with malignant mesothelioma in terms of gender, age, route of asbestos exposure, duration of asbestos exposure, pathogenic site and treatment regimens ( P>0.05 ).@*Conclusion @#The 36 patients with malignant mesothelioma had a median survival period of 0.71 years, and no association was found between the survival period and asbestos exposure or pathogenic site.

Objective @#To examine the effect of asbestos exposure on oxidative stress, so as to provide insights into the elucidation of pathogenesis and management of asbestos-related diseases. @*Methods @#Totally 245 subjects were recruited from an asbestos manufacturing area in Zhejiang Province, and their gender, age and history of asbestos exposure were collected through a questionnaire survey. The serum levels of 8-hydroxy-2'deoxyguanosine ( 8-OHdG ), glutathione ( GSH ), malondialdehyde ( MDA ), superoxide dismutase ( SOD ) and total antioxidative capacity ( TAOC ) were measured using an enzyme-linked immunosorbent assay ( ELISA ), and the levels of catalase ( CAT ), peroxiredoxin 2 ( PRX2 ), SOD1, SOD2 and thioredoxin-1 ( TRX1 ) were detected in peripheral white blood cells ( WBCs ) using a liquid-chip assay. Multivariable linear regression analysis was performed to identify the association between asbestos exposure and oxidative stress parameters. @*Results @#There were 50 subjects without a history of asbestos exposure (unexposed group), 102 subjects with asbestos exposure for less than 10 years ( AE<10-year group ) and 93 subjects with asbestos exposure for 10 years and more ( AE≥10-year group ). No significant differences were found among the three groups in terms of age, gender, proportion of smokers or proportion of alcohol consumers ( P>0.05 ). Significantly higher 8-OHdG and MDA in serum, and higher PRX2 in peripheral WBCs were detected in the AE≥10-year group than in the unexposed group ( P<0.05 ); lower GSH and TAOC in serum, and lower CAT in peripheral WBCs were detected in the AE≥10-year group than in the unexposed group ( P<0.05 ); higher 8-OHdG and MDA in serum, and higher PRX2 in peripheral WBCs were detected in the AE≥10-year group than in the AE<10-year group ( P<0.05 ). Multivariable linear regression analysis showed that asbestos exposure significantly correlated with 8-OHdG, MDA and TAOC in serum, and CAT and PRX2 in peripheral WBCs ( P<0.05 ). @*Conclusion @#Asbestos exposure may induce the oxidative stress damage, suggesting that oxidative stress may be involved in asbestos-related diseases.

ABSTRACT

Objective: To analyze the radiological characteristics of chest high-resolution computed tomography (HRCT) of patients with asbestosis, and to investigate the signs of predicting the disease progression of asbestosis. Methods: A prospective method was used to enroll 68 patients with asbestosis who were regularly followed up from 2013 to 2016. The radiological characteristics of patients with asbestosis were described by the International Classification of HRCT for Occupational and Environmental Respiratory Diseases (ICOERD) , and the differences between patients with and without progression were compared during the observation period. The Cox proportional hazards regression model was used to analyze the chest HRCT radiological signs predicting the progression of asbestosis. Results: The study included 68 patients with asbestosis aged (65.5±7.8) years old, of which 64.7% (44/68) were female, 29.4% (20/68) had a history of smoking. There was no significant difference in age, sex, smoking and asbestos exposure between patients with progressive asbestosis (20.6%, 14/68) and patients without progressive asbestosis (79.4%, 54/68) (P>0.05) . Chest HRCT of patients with asbestosis showed irregular and/or linear opacities, of which 5.9% (4/68) were accompanied by honeycombing. Irregular and/or linear opacities were mainly lower lung preponderant, often accompanied with ground glass opacity and mosaic perfusion. 98.5% (67/68) had pleural abnormalities, of which 39.7% (27/68) had diffuse pleural thickening with parenchymal bands and/or rounded atelectasis. The analysis of multivariable Cox proportional hazard regression showed that the risk of the progression of asbestosis was increased with higher irregular and/or linears opacities cores (HR=1.184, 95%CI: 1.012-1.384, P=0.034) and the appearance of honeycombing (HR=6.488, 95%CI: 1.447-29.097, P=0.015) . Conclusion: The irregular and/or linear opacities scores and honeycombing on chest HRCT are independent influencing factors for predicting the disease progression of asbestosis.

ABSTRACT

El mesotelioma es considerado en el mundo industrializado a consecuencia de la exposición ocupacional a fibras de asbesto. A nivel país se considera una enfermedad profesional. El objetivo del presente trabajo fue conocer y describir casos de mesotelioma notificados en Uruguay entre los años 2002 y 2014, con énfasis en los aspectos de la exposición ocupacional. El presente trabajo corresponde a un estudio descriptivo retrospectivo, a partir de los casos notificados se recrearon historias médicas enlazando con datos de servicios asistenciales. Se identificaron fuentes de exposición al asbesto en diferentes ocupaciones e industrias en el país. Resultados: fueron notificados 122 casos. Se accedió a la historia clínica en un tercio (47/122). El dato ocupación estaba consignado solo en 27/47, en 3/47 se explicitaba la exposición al asbesto/amianto. Los sectores productivos identificados mayoritariamente correspondieron a transporte, metalúrgico, construcción y limpieza. Se evidenció un registro insuficiente del dato ocupación y de los antecedentes laborales. Ésta información laboral es fundamental para establecer el nexo causal de la exposición en estudio y la condición de enfermedad profesional. La gravedad de la enfermedad y el conocimiento del riesgo derivado de la exposición, laboral, justifica el desarrollo de políticas en salud ocupacional. Es necesario fortalecer la formación de los profesionales de la salud sobre la importancia del trabajo como determinante del proceso salud - enfermedad.


Mesothelioma is considered in the industrialized world as a consequence of occupational exposure to asbestos fibers - asbestos. At the country level it is considered an occupational disease. The objective was to know and describe cases of mesothelioma notified in Uruguay between the years 2002 and 2014, with emphasis on aspects of occupational exposure. The present work corresponds to a retrospective descriptive study, from the reported cases medical records were recreated linking with data from healthcare services. Sources of asbestos exposure were identified in different occupations and industries in the country. Results: 122 cases were notified. The medical history was accessed in one third (47/122). The occupation data was only in 27/47, in 3/47 the exposure to asbestos / asbestos was specified. The productive sectors identified mainly corresponded to transportation, metallurgy, construction and cleaning. Insufficient registration of occupation and employment history was evidenced. This work information is essential to establish the causal link between the exposure under study and the occupational disease condition. The severity of the disease and knowledge of the risk derived from exposure occupational, justify the development of occupation health policies. It is necessary to strengthen the training of health professionals on the importance of work as a determinant of the health - disease process.


O mesotelioma é considerado no mundo industrializado como consequência da exposição ocupacional às fibras de amianto - o asbesto. Em nível nacional, é considerada uma doença ocupacional. O objetivo foi conhecer e descrever os casos de mesotelioma notificados no Uruguai entre os anos de 2002 a 2014, com ênfase nos aspectos de exposição ocupacional. O presente trabalho corresponde a um estudo descritivo retrospectivo, a partir dos casos relatados, prontuários médicos foram recriados vinculando-os a dados de serviços de saúde. Fontes de exposição ao amianto foram identificadas em diferentes ocupações e indústrias no país. Resultados: foram notificados 122 casos. O histórico médico foi acessado em um terço (47/122). Os dados de ocupação foram apenas em 27/47, em 3/47 foi especificada a exposição ao amianto / amianto. Os setores produtivos identificados corresponderam principalmente a transportes, metalurgia, construção e limpeza. Foi evidenciado registro insuficiente de ocupação e histórico de empregos. Essas informações de trabalho são essenciais para estabelecer o nexo causal entre a exposição em estudo e a condição de doença ocupacional. A gravidade da doença e o conhecimento do risco decorrente da exposição ocupacional, justificam o desenvolvimento de políticas de saúde ocupacional. É preciso fortalecer a formação dos profissionais de saúde sobre a importância do trabalho como determinante do processo saúde - doença.

RESUMO Objetivo: Mensurar a sensibilidade e especificidade das notificações de mesotelioma maligno do Registro de Câncer Base Populacional de Curitiba/Paraná. Método: Estudo retrospectivo, transversal, realizado no registro de câncer de base populacional de Curitiba e em 11 fontes notificadoras, de janeiro a dezembro de 2017. A amostra final foi composta por 92 prontuários de pacientes adultos, com diagnóstico e notificação de registrados com câncer de topografia C38 (pleura, coração e mediastino) e C48 (peritônio e retroperitônio). Para análise da sensibilidade e especificidade utilizou-se o software Stata 14, com análise da consistência interna; para cada caso notificado pelo registro foi examinado o prontuário da fonte notificadora correspondente, considerando-o como padrão ouro. Resultados: A sensibilidade do registro em notificar mesotelioma (CID-10: C45) e câncer de pleura (CID-10: C38.4) foi de 100% (8/8) e 50% (1/2), respectivamente. A especificidade foi 90,2% (74/82), tendo oito casos de outros cânceres classificados como câncer de pleura. Conclusões: O registro apresentou alta sensibilidade para notificar mesotelioma maligno, não havendo subnotificação para esses casos, e uma sobre-estimativa para as notificações de câncer de pleura (nove ao invés de um) devido à notificação errônea e equívoca.

Método: estudio retrospectivo, transversal, realizado en el registro de cáncer de base poblacional de Curitiba y en 11 fuentes notificantes, de enero a diciembre de 2017. La muestra final fue compuesta por 92 registros médicos de pacientes adultos, con el diagnóstico y la notificación de registrados con cáncer de topografía C38 (pleura, corazón y mediastino) y C48 (peritoneo y retroperitoneo). Para el análisis de la sensibilidad y especificidad se utilizó el software Stata 14, con análisis de la consistencia interna. Para cada caso notificado por el registro, fue examinado el registro médico de la fuente que ha notificado, considerándolo como estándar de oro. Resultados: la sensibilidad del registro para notificar mesotelioma (CID-10: C45) y cáncer de pleura (CID-10: C38.4) fue de 100% (8/8) y 50% (1/2), respectivamente. La especificidad fueel90,2% (74/82), con ocho casos de otros cánceres clasificados como cáncer de pleura. Conclusiones: el registro presentó alta sensibilidad para notificar mesotelioma maligno, no existiendo subnotificación para estos casos, y un incremento para las notificaciones de cáncer de pleura (nueve al revés de uno) debido a la notificación incorrecta y equivocada.

Method: Retrospective, cross-sectional study conducted in the Population-Based Cancer Registry in Curitiba and 11 reporting institutions, from January to December 2017. The final sample was composed of 92 medical records of adult patients, with diagnoses and reporting of cancer, with topographical codes C38 (pleura, heart, and mediastinum) and C48 (peritoneum and retroperitoneum). Stata 14 was used to analyze sensitivity and specificity, and internal consistency. Each medical record (considered the gold standard) provided by the reporting institutions was compared to those in the registry. Results: The registry sensitivity in reporting mesothelioma (ICD-10: C45) and pleural cancer (ICD-10: C38.4) was 100% (8/8) and 50% (1/2), respectively. Specificity was 90.2% (74/82), as eight cases were wrongly classified as pleural cancer. Conclusions: the registry presented high sensitivity in the reporting of malignant mesothelioma, with no underreporting, and overestimated pleural cancer reporting (nine instead of one) due to erroneous and misleading reporting.

ABSTRACT

RESUMO: Objetivo: Estimar o grau de concordância e validade dos diagnósticos de neoplasias malignas relacionadas à exposição ao asbesto registrados no Sistema de Informação Hospitalar do Sistema Único de Saúde (SIH/SUS), em comparação aos Registros Hospitalares de Câncer do Estado de São Paulo (RHC/SP). Métodos: Óbitos com registros de neoplasias malignas associadas ao asbesto foram identificados e extraídos do SIH/SUS entre 2007 e 2014. Óbitos nos casos de câncer registrados na base do RHC/SP foram extraídos para o mesmo período. Essas bases foram unidas pelos mesmos indivíduos empregando-se o software Link Plus. Um único diagnóstico codificado pela CID-O3 ou CID-10 selecionado de cada sistema foi analisado. A proporção de concordância e a sensibilidade, especificidade e valores preditivos foram estimados. Resultados: Encontraram-se 19.458 pares com registros nas duas bases. A proporção de concordância foi elevada, variando de 92,4% para a localização primária desconhecida a 99,7% para o câncer de pleura. O índice Kappa variou de 0,05 (IC95% 0,04 - 0,07) para o câncer de pleura a 0,85 (IC95% 0,84 - 0,87) para o câncer de pulmão. A menor sensibilidade foi de 0,08 (IC95% 0,01 - 0,25), para o câncer de pleura, e a maior de 0,90 (IC95% 0,90 - 0,91), para o câncer de pulmão. Conclusão: Diagnósticos de neoplasias malignas associadas ao asbesto alcançaram maiores níveis de concordância e validade quando comuns. Os diagnósticos mais raros apresentaram baixa acurácia no SIH/SUS.


ABSTRACT: Objective: To estimate the degree of agreement and validity of diagnoses of asbestos-related malignant neoplasms registered in the Hospital Information System of the Brazilian Unified Health System (SIH/SUS), in comparison to the Hospital Cancer Registries of the State of São Paulo (HCR/SP). Methods: Deaths with records of malignant neoplasms associated with asbestos were identified and extracted from SIH/SUS between 2007 and 2014. Deaths in cases registered in the HCR/SP were extracted for the same period. The databases were linked using software Link Plus. A single ICD-10-coded diagnosis selected from each system was analyzed. The proportion of agreement, and the sensitivity, specificity and predictive values were estimated. Results: 19,458 pairs were found with records in both bases. The proportion of agreement was high, ranging from 92.4% for the unknown primary site, to 99.7% for cancer of the pleura. The Kappa Index ranged from 0.05 (95%CI 0.04 - 0.07) for cancer of the pleura to 0.85 (95%CI 0.84 - 0.87) for lung cancer. Sensitivity varied from 0.08 (95%CI 0.01 - 0.25) for cancer of the pleura, to 0.90 (95%CI 0.90 - 0.91) for lung cancer. Conclusion: Diagnosis of asbestos-related malignancies reached higher levels of agreement and validity when common. Rare diagnoses showed low accuracy in SIH/SUS.

ABSTRACT

OBJECTIVE: To analyze the clinical manifestations and diagnostic points of patients with peritoneal mesothelioma caused by occupational asbestos. METHODS: The clinical data of a female patient with peritoneal mesothelioma caused by occupational asbestos and the diagnosis of occupational diseases were retrospectively analyzed. RESULTS: In 2016, the patient was diagnosed and treated in a number of hospitals in a province due to chest and back pain, persistent cough, suffocation, and foamy sputum. After laparoscopic surgery, the peritoneal biopsy was taken for pathological analysis and diagnosed as peritoneal mesothelioma. In December 2016, she died due to a worsening of her condition and lung infection. The patient′s family requested occupational disease diagnosis in May 2017. After investigation and verification by the local occupational disease diagnosis agency and the Bureau of Industry and Information Technology, it was clear that the patient had a history of occupational exposure to asbestos for a total of 23 years and two months. In July 2018, she was retrospectively diagnosed as an occupational tumor(mesothelioma caused by asbestos). CONCLUSION: A clear history of occupational exposure to asbestos and histopathological examination are helpful for the diagnosis of occupational tumors(mesothelioma caused by asbestos).

BACKGROUND@#Asbestos fibers possess tumorigenicity and are thought to cause mesothelioma. We have previously reported that exposure to asbestos fibers causes a reduction in antitumor immunity. Asbestos exposure in the mixed lymphocyte reaction (MLR) showed suppressed induction of cytotoxic T lymphocytes (CTLs), accompanied by a decrease in proliferation of CD8@*METHODS@#For MLR, human peripheral blood mononuclear cells (PBMCs) were cultured with irradiated allogenic PBMCs upon exposure to chrysotile B asbestos at 5 μg/ml for 7 days. After 2 days of culture, IL-15 was added at 1 ng/ml. After 7 days of MLR, PBMCs were collected and analyzed for phenotypic and functional markers of CD8@*RESULTS@#IL-15 addition partially reversed the decrease in CD3@*CONCLUSION@#These findings indicate that CTLs induced upon exposure to asbestos possess dysfunctional machinery that can be partly compensated by IL-15 supplementation, and that IL-15 is more effective in the recovery of proliferation and granzyme B levels from asbestos-induced suppression of CTL induction compared with IL-2.

INTRODUCCIÓN: Las placas pleurales por fibras de asbesto se caracterizan por lesiones compuestas por tejido fibroso que se ubican en la pleura parietal. Suelen aparecer hasta en un 3% a 58% de los trabajadores que estuvieron expuestos a fibras de asbesto, y en un 0,5% a 8% en población general. El OBJETIVO de este artículo es presentar dos casos clínicos de pacientes a los que se les detectó en la radiografía de tórax alteraciones pleurales posiblemente asociadas a la exposición a fibras de asbesto. CASOS CLÍNICOS: Caso 1. Paciente de sexo masculino de 49 años, trabajador de la construcción con antecedentes de exposición a fibras de asbesto. Se le realizó una radiografía de tórax con técnica OIT (Organización Internacional del Trabajo), la que evidenció alteraciones pleurales focales. Una tomografía axial computarizada de tórax, confirmó la presencia de placas pleurales. Caso 2. Paciente de sexo femenino de 79 años, jefa de hogar, sin exposición laboral conocida a fibras de asbesto. En una radiografía de tórax anteroposterior, se observó la presencia de una placa pleural focal. Se solicitó una tomografía axial computarizada de tórax, la que confirmó la presencia de placas pleurales. CONCLUSIÓN: La radiografía de tórax con técnica OIT es el instrumento básico para la identificación de enfermedades relacionadas con la exposición a fibra de asbesto. El estudio debe ser completado con una tomografía axial computarizada de tórax cuya sensibilidad es mayor permitiendo detectar tempranamente las anomalías pleurales. Es fundamental la historia ocupacional detallada ya que constituye el método más fiable y práctico para medir la exposición a fibra de asbesto.


INTRODUCTION: Asbestos fiber pleural plaque is characterized by lesions composed of fibrous tissue that are located in the parietal pleura. They usually appear in up to 3 to 58% of workers who were exposed to asbestos fiber, and 0.5 to 8% in the general population. The OBJECTIVE of this article is to present two clinical cases of patients who were detected in the chest radiograph pleural alterations associated with exposure to asbestos fibers. CLINICAL CASES: First case: 49-year-old male patient, building worker with a history of exposure to asbestos fibers. Focal pleural alterations were detected by a chest x-ray performed according ILO (International Labour Organization) technique. The presence of pleural plaques was confirmed in a computed tomography of the chest. Second case: Holder, without occupational exposure to asbestos fibers. An anteroposterior chest radiography showed the presence of focal pleural plaque in a CT scan of the chest. CONCLUSION: Chest x-ray with ILO technique is the basic instrument for the identification of diseases related to asbestos fiber exposure. The study should be completed with a CT scan of the chest whose sensitivity is greater, allowing early detection of pleural abnormalities. Detailed occupational history is essential, as it is the most reliable and practical method to measure asbestos fiber exposure.

ABSTRACT

Benign multicystic peritoneal mesothelioma (BMPM) is a rare peritoneal tumor diagnosed predominantly in pre-menopausal women. Associated risk factors include endometriosis and pelvic inflammatory disease in women, and prior abdominal surgery in both genders. To date, the pathogenesis of this disease remains controversial with possible etiologies, including a neoplastic versus a reactive process. Given the risk factors, some authors believe that this disease is secondary to a reactive process. However, because some studies describe cases where there is no prior surgical history or inflammatory milieu present, and because of this entity's predilection for recurrence, some authors believe the origin to be neoplastic. Some genetic and familial associations have also been reported. Malignant transformation is extremely rare, with only two cases reported in the literature, despite the recurrence potential. Like the etiology, the name of this entity is also controversial. Some authors prefer the term "peritoneal inclusion cyst (PCM)" instead of "benign cystic mesothelioma" and argue that the term mesothelioma should only be used when there is evidence of atypia. Most cases of BMPM are discovered incidentally. Others reflect sequela of tumor mass effect. It appears intra-operatively as large, multi-focal, cystic lesions in the peritoneal and pelvic cavity. Diagnosis is achieved through surgical sampling with histopathological examination. Immunobiologically, BMPM exhibits multiple small cystic spaces with flattened lining containing calretinin positive cells without atypical features, mitotic figures, or tissue invasion. Treatment includes cytoreductive surgery. Here we present a case of BMPM in a 60-year-old male - a rare disease in an uncommon patient population.

ABSTRACT

Asbestos exposure is known to cause malignant mesothelioma, which is associated with poor prognosis. We focused on and examined the effect of asbestos exposure on the differentiation and function of cytotoxic T lymphocytes (CTLs). CTLs have the ability to specifically attack tumor cells after being differentiated from naïve CD8 T cells following antigen stimulation. Exposure to chrysotile B asbestos suppressed the differentiation of CTLs during the mixed lymphocyte reaction (MLR) and was associated with a decrease in proliferation of CD8 T cells. Additionally, in an effort to investigate the mechanism associated with suppressed CTL differentiation upon exposure to asbestos, we focused on IL-2, a cytokine involved in T cell proliferation. Our findings indicated that insufficient levels of IL-2 are not the main cause for the suppressed induction of CTLs by asbestos exposure, although they suggest potential improvement in the suppressed CTL function. Furthermore, the functional properties of peripheral blood CD8 lymphocytes from asbestos-exposed individuals with pleural plaque (PP) and patients with malignant mesothelioma (MM) were examined. MM patients showed lower perforin levels in CD8 lymphocytes following stimulation compared with PP-positive individuals. The production capacity of IFN-γ in the MM group tended to be lower compared with healthy volunteers or PP-positive individuals. In an effort to determine whether chronic and direct asbestos exposure affected the function of CD8 T cells, cultured human CD8 T cells were employed as an in vitro model and subjected to long-term exposure to chrysotile (CH) asbestos. This resulted in decreased levels of intracellular perforin and secreted IFN-γ. Those findings underlie the possibility that impaired CD8 lymphocyte function is caused by asbestos exposure, which fail to suppress the development of MM. Our studies therefore reveal novel effects of asbestos exposure on CTLs, which might contribute towards the development and implementation of an effective strategy for the prevention and cure of malignant mesothelioma.

OBJECTIVE: To retrospectively analyze the clinical characteristics of 5 cases of occupational asbestos-induced pleural mesothelioma. METHODS: The clinical features and occupational diagnostic data of 5 patients with pleural mesothelioma caused by occupational asbestos exposure were collected. The occupational history, the occupational hazard exposure history, the length of latency, the clinical features, chest computed tomography(CT) and immunohistochemical staining examination of these 5 patients were analyzed. RESULTS: The median of occupational asbestos exposure length was 8 years and the median of latent period was 30 years in these 5 patients. One patient had been diagnosed as occupational asbestos lung. Typical chest CT findings of asbestos-induced pleural mesothelioma were one-sided pleural effusion, pleural nodules and enlarged mediastinal lymph nodes. The clinical symptoms were mainly chest pain, cheat tightness, and fatigue. Erythrocyte sedimentation rate increased in 3 cases. According to immunohistochemical findings, 4 patients showed that melanocortin, calretinin, cytokeratin 5/6, and cytokeratin 7 were positive, and 3 patients showed that wilms′ tumor nuclear protein were positive. Examination of lung function in 4 patients showed restrictive ventilation dysfunction. CONCLUSION: Most patients with pleura mesothelioma showed insignificant symptoms at early stage. The occupational health surveillance and chest CT examination of occupational asbestos-exposed workers should be strengthened, and early diagnosis should be made as soon as possible to win surgical opportunities.

ABSTRACT

Abstract The usefulness of an industrial product must be measured both in economic profitability and product safety terms, and the social benefit it represents. In the case of asbestos, due to its harmful effects on human health, its use, handling and production has been banned in high-income countries thanks to the efforts carried out by their oversight bodies. Worldwide, the industrial use of this mineral has been associated with high morbidity and mortality rates, hence the importance of denouncing the health effects of asbestos. Asbestos is a term used to refer to six naturally occurring silicate minerals that are used in the manufacture of building materials, such as asbestos-cement, and automotive components, including brake linings and brake pads; however, it has been proven that inhaling asbestos microscopic fibers can lead to the development of lung diseases (pneumoconiosis) and cancer. In many cases, these diseases are caused by a short occupational or environmental exposure to it, but their clinical manifestation occurs several years after the first time of exposure. The main objective of this paper is to reflect on the hazards related to the use of asbestos and to influence public health policies addressing this problem in Colombia, so that by means of the newly adopted law banning the use of this mineral in our country, significant progress is made in aspects such as the identification and the monitoring of people who were exposed to it, and the handling, removal and final disposal of materials containing asbestos.


Resumen La utilidad de un producto industrial se debe medir tanto por su rentabilidad económica, como por su seguridad y beneficio social. En el caso del asbesto, debido a sus efectos adversos sobre la salud, su uso, manipulación y producción se ha prohibido en países de altos ingresos gracias a esfuerzos realizados por sus respectivos entes de control. A nivel mundial, el uso industrial de este mineral se ha asociado con altas tasas de morbimortalidad, de ahí la importancia de denunciar sus efectos para la salud. El asbesto comprende un grupo de seis minerales fibrosos de origen natural que la industria utiliza en la fabricación de materiales de construcción en fibro-cemento y frenos de automóviles; sin embargo, se ha demostrado que la inhalación de sus fibras microscópicas puede producir enfermedades pulmonares (neumoconiosis) y cáncer. En muchos casos, estas enfermedades son causadas por una corta exposición laboral o ambiental al material y se manifiestan años después. El principal objetivo de este documento es reflexionar sobre los peligros del uso del asbesto e influir en las políticas de salud pública al respecto, esto para que con la recién aprobada ley que prohíbe el asbesto en Colombia se logren avances significativos en temas como la identificación y el seguimiento de las personas que estuvieron expuestas, y el manejo, remoción y eliminación de los productos que contienen el mineral.

El mesotelioma difuso es una neoplasia maligna poco frecuente que proviene de las células mesoteliales; la pleura es su localización más habitual y se estima que un 10% de los casos se localizan a nivel peritoneal. El mesotelioma es habitualmente diagnosticado en la quinta década de la vida, con un claro predominio en el sexo masculino, debido esto último a su relación con la exposición laboral al asbesto. Resumo: Introdução: Os asbestos são minerais compostos por fibras naturais que foram muito utilizados na indústria devido suas propriedades físico-químicas que lhe garantem a exposição às altas temperaturas sem sofrer alteração. Existem dois tipos de fibras sendo que somente as serpentinas (crisotila) ainda são extraídas e comercializadas em alguns países. Contudo, se por um lado, as serpentinas trouxeram facilidades para a civilização, por outro, geraram dilemas conflituosos acerca de sua toxicidade no ambiente e na saúde das pessoas. Objetivo: Buscar evidências na literatura científica sobre a mortalidade por câncer entre a população adulta e idosa que foram submetidas à exposição cumulativa aos asbestos crisotila no ambiente ocupacional, paraocupacional ou ambiental. Metodologia: Trata-se de uma revisão sistemática de literatura. Para tanto, utilizaram-se os critérios de revisão sistemática estabelecidos pelo Instituto Joanna Briggs para identificar o nível de evidência - prognóstico; assim, os estudos do tipo coorte sem limite de data, nos idiomas português, inglês, alemão, espanhol e italiano, foram exclusivamente analisados. As bases de dados para a averiguação dos estudos foram a Medline/Pubmed, Cinahl, ScienceDirect, Scielo, Web of Science, The Cochrane Library, Lilacs e Scopus. Resultados: O protocolo desta revisão sitemática foi submetido ao PROSPERO sob número de registro CRD 42018095749. Recuperados 1643 artigos que após a análise inicial, resultou em 139 artigos. Destes, quatro foram selecionados para identificar a relação entre a mortalidade por câncer e a exposição ao crisotila puro. Devido à heterogeneidade das informações, não foi possível estabelecer a metanálise, portanto, procedeu-se à síntese narrativa dos dados. Discussão: Os estudos de coorte apresentaram fragilidades metodológicas que impossibilitaram estabelecer o nível de evidência de que a mortalidade por câncer está aumentada na população exposta ao crisotila. Porém, ainda que a evidência científica não tenha sido definida com esta revisão sistemática, destaca-se que há indícios de que tal hipótese possa ser verdadeira. Considerações: Ressaltamos sobre a importância dos estudos de coorte seguirem rigorosamente as diretrizes de qualidade para descrever as informações necessárias que viabilizem ao leitor avaliar o estudo e utilizar os dados de forma confiável. Recomendamos maiores investimentos em pesquisas na área, com descrição metodológica rigorosa e detalhada do estado vital dos participantes, concentração e natureza das fibras inaladas, tempo de exposição com especificação em horas por dia, local da exposição, diagnóstico confirmado e notificado compulsoriamente em sistemas competentes nos países de origem, e medidas de prevenção de fatores de confusão para que se estabeleça o crisotila puro como nexo causal do aumento de mortalidade por câncer. Todavia, enquanto não se estabelecer a evidência, é imprescindível que haja esforços no sentido de administrar recursos para a prevenção e o tratamento das doenças relacionadas aos asbestos.


Abstract: Introduction: Asbestos are minerals composed of natural fibers that have been widely used in the industry due to their physicochemical properties that guarantee exposure to high temperatures without alteration. There are two types of fibers and only the serpentines (chrysotile) are still extracted and marketed in some countries. However, while on the one hand serpentines have brought facilities for civilization, on the other, they have generated conflicting dilemmas about their toxicity to the environment and people's health. Objective: To find evidence in the scientific literature on cancer mortality among the adult and elderly population that were submitted to cumulative exposure to asbestos chrysotile in the occupational, para-occupational or environmental environment. Methodology: This is a systematic literature review. To do so, the systematic review criteria established by the Joanna Briggs Institute were used to identify the level of evidence - prognosis; therefore, the cohort studies with no date limit in the Portuguese, English, German, Spanish and Italian languages were exclusively analyzed. The databases for the investigation of the studies were Medline / Pubmed, Cinahl, ScienceDirect, Scielo, Web of Science, The Cochrane Library, Lilacs and Scopus. Results: The protocol of this sitematic review was submitted to PROSPERO under registration number CRD 42018095749. Recovered 1643 articles that after the initial analysis, resulted in 139 articles. Of these, four were selected to identify the relationship between cancer mortality and exposure to pure chrysotile. Due to the heterogeneity of the information, it was not possible to establish the meta-analysis, therefore, the narrative synthesis of the data was done. Discussion: Cohort studies showed methodological weaknesses that made it impossible to establish the level of evidence that cancer mortality is increased in the population exposed to chrysotile. However, although the scientific evidence has not been defined with this systematic review, it is emphasized that there is evidence that such a hypothesis may be true. Considerations: We emphasize the importance of cohort studies to strictly follow the quality guidelines to describe the necessary information that will enable the reader to evaluate the study and use the data reliably. We recommend greater investments in research in the area, with a detailed and detailed methodological description of participants' vital status, concentration and nature of inhaled fibers, time of exposure with specification in hours per day, location of exposure, diagnosis confirmed and reported compulsorily in competent systems in countries of origin, and measures to prevent confounding to establish pure chrysotile as a causal link to the increase in cancer mortality. However, as long as the evidence is not established, efforts must be made to administer resources for the prevention and treatment of asbestos-related diseases.
